AMD + NS2 is currently a no go, this is an AMD driver bug.
AMD has been made aware and we're currently waiting for a fix or UWE might have a workaround in the future

With Nvidia it works last I heard, on AMD drivers it's broken.
Haven't tried with the open source AMD drivers yet. -

Friend of mine runs those without issue, except for then the mods update and the map/server hasn't cycled yet.
Could you check if verifying files on the server makes any difference? -

I have the same, have to fiddle with alt+tab (switch window) and ctrl+shift+esc (taskmanager) to get proper focus.
Just alt tabbing leaves me with NS2 showing on primary monitor but not in focus, once I click it switches. -

Tested guide with build 260, apart from config folder already existing it works.
Edit: after deleting the old attachment I couldn't find a way to attach the new file.. -
