I know if there's a thread about games i bring this one up, but i'm going to again.
Project 0 / Fatal Frame. The whole game is freaky. There's not a single moment when you actually feel at ease.
The scariest moments (for me) have to be either when you see a woman in a kimono under the building (in the cherry atrium) and she trys to grab at you and then disappears.
Or the first time you see the floating female head. That has actually scarred me for life. When playing the game i hand over the controller to my friend because i can' t bear to play it when i'm in that area. It's actually the scariest thing i've ever seen in a game. It just floats through the wall right infront of you and starts going for your face. It's so fast. *shudder*
The whole game is immensely tense and there's so many scary parts. I found myself getting shivvers down my spine every time i would fight a ghost.
Also, as i stressed in another thread, in my opinion, when it comes to overal atmosphere and fear, SS2 (as excellent as it is) doesn't hold a candle to that of Project 0. Same goes for Resident Evil 1-385697239785r32.

Well, all mine have been in Half life, or AVP-AVP2
Half-Life (Custom single player map-cant remember name) I was crawling through a dark vent, flashlight energy gone (it was sorta like a mod, flashlights required batteries), then out of no where, 10 snarks and about 5 headcrabs attacked me. I swear I yelled to the top of my lungs, whipped out my mp5, fired all the rounds, took out the hornet gun, fired, took out my pistol, and emptied it in about 3 seconds....I died soon after.
My vote goes for System Shock 2. Its the only game that made me not want to play, but still dragged me along for the ride anyway. I was ALWAYS low on ammo, and there never seemed to be a replicator around when you need it. First time I saw a rumbler, I heard the blood curdling scream, and then saw this huge thing charging at me. AFter unloading the rest of my ammo from my assualt rifle into it, and realizing that my shotgun was empty to, I proceeded to whack it with my wrench....
I didn't survive.....
SHODAN is also the creepiest villian, just her voice a mixture of an adult female, children, mechanical, and varoius other things, is enough to give me nightmares.
scariest thing...... when i played a game, i dont no what it was called, was a mystery/1st person shooter, set in a dark house in the cornish moors, walls started movin and stuff, thing is the room i was in looked kinda like my bedroom. nearly made me soil myself, especially at 3.00 in the mornin when i've bin deprived of my favourite activity, sleep
dude the ver very begining of system shock where you come out of teh tube and if you are me skipped teh video so had no idea whats goin on and the level starts attacking me. that and the first ghost i saw in the game destroyed me i say it and i toom off runnning i jumped over the railing i was stading by bck down to the botom. that and the hanging guy all of these are in like the first 30 seconds of teh game and i was just a quivering pile of jelly. yeah..

Yea well when you either have to go into the sewers, dark cave, underground, or some really huge area filled with dark and loud machinery you know you are screwed.
Also when after going forever with low health and ammo you come to a room just FILLED with goodies. You rejoice for a second and then suddenly you stop. You start to wonder wth all this stuff is for.... usualy by now the dramatic music starts and you know your in for several retrys and quick loads.
Great, now i have the urge to crack out my SS2 cd and give it another run through. Actually, to this day i don't think i've finished the game only using the psy (or whatever they're called) weapons.
yeah, SS2 was scary and creepy and all and I love it very much aswell, ...BUT! shodans voice just plain **** me off. god damn it a freaking super AI and it can't talk straight to save it's life! took her ages to get one word over the comm, let alone a complete sentence! I would sit there and get a headache having to listen to her poor attempt at vocal communication all the while trying to stop her from becoming god! I suppose Spock would have magically experienced a few certain emotions witnessing that twisted logic. in the end her talk probably was the main reason I wanted to put as much lead as I had on me through her and not because she was just plain evil... <!--emo&:D--><img src='http://www.unknownworlds.com/forums/html/emoticons/biggrin.gif' border='0' style='vertical-align:middle' alt='biggrin.gif'><!--endemo-->
oh and on the topic of most scary gaming moment: hard to tell, but I believe the third (?) level of Dark Forces where you had to swim through sewers inhabited by dianogas (those one eyed monsters of which you could see one in the scene in the trash compactor on the death star in epIV) probably qualifies as 'very scary' in my gaming career.
