i feel like i'm the only one who doesn't seem to have any hit registry problems whatsoever. :(
of the (very limited) videos I have seen trying to demonstrate reg issues, it seems like they flat out missed or didn't hit center mass with the shotgun and had the expectation that every pellet's supposed to hit or something silly like that.

Mostly from my experiences it seems that the biggest problems are animations and general warping of players. Animations seem really bugged especially with skulk wallclimbing because the transitions arent smooth at all. When walljumping the skulk will go close to railings etc. making the animations spas out quite badly, trying to figure which way to turn the skulk. Also simply going onto a wall and moving your mouse will make the skulk spin very rapidly sometimes. Then theres the animation/turning lag. Aliens usually seem to be looking and animation wise traveling to opposite way of whats the reality, making it really hard to predict (aim) at them.
Lerk with its turning animations suffer from a lot of the same problems.
Collisions between players is a big problem as well, when close players simply keep going through each other making shots seemingly just go through as well.
Theres definitely more than just those problems as sometimes shots simply seem to disappear, but animations and warping are huge problems contributing to the problem a lot.
I wonder about it being problems with the animations also.
Was talking to a friend the other day about having to deal with marines shooting you while it looks like they are not looking at you. So say your circling around a marine, to you it looks like he is shooting a foot behind you but you're taking damage. I only play on servers that I get decent ping on, and I know others deal with the same thing on a regular basis.
While the whole jumping around trying to dodge a marine's fire is tons of fun when you can't even tell where he really is shooting I actually was curious if this is what causes the hit reg problem. For me when a marine spins, it's not like his hitbox is going to drastically change because he's facing towards me instead of facing left. But for a marine, if a skulk looks like he's facing left but really facing more towards you.. where is his hitbox?
---edit--- Talked to a friend about it and he said their hitbox is client side, so guess that's not it!
