Lower resolution and texture quality, turn off shadows, atmospherics, occlusion and AA. And only choose servers with low ping and high performance.

A slight upgrade to your video card would probably help, since you have greater then 1ms waiting for GPU that means that video card is your weakest factor. I am not sure how much of an improvement you would see however. (also obviously your video card is quite old)

A 220 GPU is very low-end. Out of the XXX number system Nvidia currently uses, the first X denotes the generation. the Second X denotes out of 9 how fast that generation is. and the last can be sub-versions of that second number.

So a for instance, the latest cards are 5XX series. 520 would be on the low end, 540-560 is mid-rage and 570+ is high-end. You're 3 generations out from the latest, and also on the low-end of that generation. I would suggest you try a new Graphics card in the 540-560 range at least.

The biggest FPS hogs are Shadows and Ambient Occlusion.

So make sure Shadows are off, and Ambient occulsion is off. Anti Aliasing should be off as well. Texture quality and resolution wont help much imo.
