Next would have to be the They Hunger s-p series, esp the first episode. This level kept me on my toes more so then HL mainly due to the errie atmosphere/setting.mmmm..brains!
then theres diablo..but it was a short one but the most memorable moment for a RPG. anyone remember THE BUTCHER? the first time i heard his hellish voice i jumped and threw the mice! (yes i was standing there in-game like a tard all that time) This always makes me smile and chuckle thinking back.

Anyway, you people are making me want to pick up a copy of system shock 2. I've never seen or played it, but it sounds intriguing. Undying was a HUGE disappointment.
Under the right circumstances, NS can give you a serious case of the willies.
Doom - I think I got the game when I was 8 or 12, I don't remember. It was scary because of all the creepy noises the demons made while fighting in the dark.
Resident Evil 2 - When I first got the game I knew nothing about the Resident Evil series. I remember flying out of my chair when zombies started grabbing me through the blockaded windows.
System Shock 2 - God damn this game scare the #### out of me many times. Those ghost you see from time to time gives me chills down my spine. I run and look for a place to hide whenever the cameras spoted me. Also the ambient noises of monsters in the distance was scary. But that wasn't why I stopped playing. I just got fustrated over the stats and stuff. Skill stats and FPS don't mix for me.
AvP and AvP2 - The marine campaigns are freaky. Almost all the levels are in the dark and the noise of my motion tracker doesn't help. The motion tracker does more harm then good. I get scared whenever I tracking something. The rapid beeping of the tracking puts me on the edge of my seat. Numerous times I go into a panic whenever one of the bugs came out of no where. I run for the nearest ladder whenever I hear facehuggers crawling around.

I've never seen a picture of her that looks good; she doesn't capture well. But her attitude and appearance on-screen and her talent as an actress are a completely different story.
And to keep this semi-on-topic, Milla kicked ### (and looked good doing it - and I don't just mean the shower scene, even!) as the lead role in the (very good) recent movie adaptation of <i>Resident Evil</i>.
