Bad Surface Extends, Etc

Every once in a while, HL crashes and gives me this message:
Bad surface extends 1424/0 at position (-1144, 1160, -992)
Also, quite often I can not join because my map "differs from server's". If I retry 5-6 times, it usually loads though.
Anyone able to clarify this a bit? I tried search but that came up dry.
I use NO 3rd party programs, standard Det drivers, etc.
This happens mostly in NS but I believe it has happened once in DoD.
