mouse4 and mouse5(side keys), the tiny buttons are mwheelup and mwheeldown, the same as when scrolling the wheel. Pressing the wheel down is mouse3. The last(tiny) button can't be bound what I am aware of. I don't use the logitech drivers, using them might not work the same way when binding keys as just pluging in the mouse but I think it should.
as for HOW to bind them, open up your config.cfg with notepad and copy one of the bind lines and edit it as neccessary, make sure you save it as config.cfg and youll have the new bind the next time you open the game.

A much simpler way to bind your keys is just to use the built in controls tab that you would normally use(it works for me and I have that exact same mouse, if it doesn't work my best guess would be that you have installed the logitech drivers and they somehow cause you not to be able to bind the buttons). You can also bind the keys by name from the console.
like so: bind keyname "command"
quotes are only nessecary for a series of commands or commands like messagemode1 and 2(the say and teamsay commands). To add several commands to a key use ; as a separator.
If you cannot bind any keys at all or change anything then your config is most likely read only.
Speaking as a owner of logitechs cordless optical mouse, I can say that logitech makes horrible drivers. When I had my mouse drivers installed, I couldn't bind the button on the left to anything. The problem was that in the drivers it was set up to be the back button when browsing the net. But however when I unintalled the drivers I could bind it like any other mouse button, AND it worked as the back button in my browser. This might be because of my own stupidity, or maby just a problem with the cordless optical mouse. So if the above methods fail, try uninstalling the drives.

you can cahnge the behaviour of the mouse four button on a logitech mouseome where in teh software it comes with you can chooe what keys are what you can make teh button be "mouse4" or what ever you like

There fore in order to play ns you have to map your mouse buttons to keys. I mapped left mouse(previously back in the web browser to pgup) and i mapped mouse right to pgdown)
Now in the ns key binding wherever you want to use mouseleft you type pgup and pgdn for mouseright. when playing you can use either pgup and mouseleft will do the same. eg (bind pgdn "impulse 100") then when u push mouse right its mapped to pgdn and ur flashy will turn on.
Yes. I'm going to presume you're using the default set up. Though setting it to ctrl is a bad idea as in this case you won't be able to move left while crouching, so I've changed it to alt. Paste this into your autoexec.cfg file, (or userconfig.cfg if you're using steam).

Hi ppl, i have Genius Net Scroll Optical USB (4 buttons + wheel) and a diskette with drivers. What i do? I dont install them!(dont have floppy drive in my comp anyway, its sitting at the very deep of some cardboard box) then i can bind any of the buttons normally in HL/NS and other games (most of them, mainly newer ones).

So just hold ALT and and press 'A' and you'll get the adrenaline upgrade, release ALT and the 'A' key will be rebound.
