I had this problem also. Entities are in two places. Info_randomteam and all other team joins are accessed by clicking on the entitie tool on the left menu, it looks like a chess pawn. Then scroll through the drop down list. To make a func_ladder, make a ladder structure, then click on it so its selected, then click on "to entitie" on the right side menu, and scroll through there. I thought they were missing for me to, but I didnt know where to look for them. Use the search option on these forums. Most of the answers are already here.

The difference is point-based versus brush-based entities. A brush-based entity cannot exist indepently of a physical brush of the same sort you build rooms with; this is because a brush-based entity has some effect that is linked to a physical volume. In the case of info_randomteam etc, it is brush-based because it will cause you to join a random team when you step inside the brush.

Point-based entities exist at some specific point in the map and are not linked to brushes at all - 'light' is a good example of a point-based entity, it just casts light out from that specific point.

There are numerous tutorials that cover the basics of half-life engine mapping; you should look them up. Try 69th vlatitude, valve-erc, etc.
