I've had this happen to me once before, and not as a fade.
My theory is that it's something about teleporting to a new position in the world mixed with high latency = occasional inaccurate position placement, where you are half in, half out.

This had been around since before comp mod. It's pretty rare, but obviously game-ruining in competitive, I had only seen it happen in a scrim. I egg right by that spot all the time and it's never happened to me.
If you can't reproduce it, is there a way to make /stuck work in the tunnel? I feel like it doesn't work in there, I don't recall
