Here's the fixed editor_setup.xml. Don't thank me for the fix, because I didn't fix it, but I am not about to throw under the bus whoever broke/fixed ladders.
Note, if you have saved your map since 268 dropped, your ladders WILL BE BROKEN. You need to re-scale the ladder entity to get it to work again.

'Join team' entities are back to a little cube.
A nice new color BTW.
Seems something was forgotten in this new patch.
Death triggers also, but that should all be fixed in that XML.
Could you make a "diff" (linux shell) on the file and an original (fresh install) ?
I already modified mine accordingly to what was suggested by @SamusDroid.
