Steam workshop issues

Yes yes we all know that steam workshop works about as well as a 10-year old lighting his farts: sometimes it works, but often times things don't go so well and you have to vent out the house for a few weeks.
Anyways, what's the cause for this? Obviously ns2 isn't the only game that works with steam workshop, but I barely play any other games, let alone make new content for other games. The only other brush with steam workshop I have is in Company of Heroes 2, and it always seems to work just fine, all the time. Has anybody else had any experience with steam workshop outside of ns2?
Second question, regardless of the source, is there anything being done to try to make the steam workshop more reliable?

Also, do game developers have to pay to incorporate Steam Workshop in their game... or how does that work?
