Booted to main menu while in game

Since patch 236, I've been regularly booted from the middle of a game to the main menu with an error that says "Client and Server differ". I was hoping patch 237 would fix it, but it's still popping up.
Note that this isn't happening when loading or trying to join a server. This only occurs when I'm in the game already, after playing for at least 10-20 minutes.
Checked the logs, and this was at the bottom:
