    And don't use carve, it's an optimisation killer.

    Yep, you can either used four separate brushes (walls) to make the four sides of the wall around the hole, or...

    you can also use the 'Clip Tool'. Draw a line across the brush (the wall), when its selected, and you're on the Clip Tool. The red shows you what will be deleted if you hit Enter. But, if you click the Clip Tool again before hitting Enter, the Red part actually flips, so the other side will be deleted. Hit it again, and you can avoid deleting either side; instead, you cut the two pieces apart right along that line. From there, you can cut them again the other way, and you suddenly turned one solid wall into four, which can then be maneuvred to have an opening in the middle; and you have your hole.
