Perhaps evil cs changed voice_enable to 0 ? Be sure to check this, must be 1 to get your mic working.

Check your hardware .. make sure it is plugged in correctly .. and securely.

    Then type:

    voice_enable 1 in the console.

My mic also doesn't work, and fiddling with voice_enable, voice_dsound, and a few others have produced no results. I am also certain that the mic is working and is plugged in. If it helps, my soundcard is an ESS Allegro-1.

try typing "voice_loopback 1" in the console without quotes.

I think voice_loopback lets you hear what you're transmitting (your name appears as if you're speaking), but I sound like a very faint mumbling, regardless of what I am (or am not) saying.
