  use 'Weld Selected' under vertex mode on the Edit Mesh modifier. Youll probably have to bump up the value unless theyre right on top of eachother.

  Select the verticies you want to weld and press the collapse button.
  
  Or under the welding options choose target with one vertex selected and drag it on to the other vertex.
