Up for forward, Back for Backwards, Left for Strafe left, Right for Strafe right. (simple eh?) Right-Shift is jump Right-Control is duck Enter is Use Mouse1 for fire Mouse3 for reload
As needed I assign things to my delete, end, insert, home, \ backspace. I also use my numberpad for weaponswitching (I'd use mousewheel if it worked), and my numberpad0 for voicecomm.
In contrast, I use WASD for when I'm COMMing, because for some reason, the arrow keys don't work correctly.
Most people don't like my key setup, but if I try to play using WASD at fast speed, I end up nailing the CAP LOCK, Q, E, Z, X and several other keys in that area instead...for me, the arrowkeys are just a lot less cluttered, and make quick movement easier. <!--emo&:)--><img src='http://www.unknownworlds.com/forums/html/emoticons/smile.gif' border='0' style='vertical-align:middle' alt='smile.gif'><!--endemo-->

I've been using this control scheme since Deus Ex, and I've yet to improve on it.

Foward: Mouse 2. Been doing it in every fps since half life deathmatch.
My other directionals are Back A Left Strafe D Right Strafe F
The way my controls are set, I can still used the number keys for quick switching of weapons i.e. leap-bite combos, and my left hand doesn't get all contorted after many hours of gaming like a wasd combo would do.
Shift is crouch Space is jump E use R reload Q walk W flashlight
S is my secondary fire in any mod/game that has such. For my command menu, it's mouse3.
There was a thread a little while ago where I guess I found out I wasn't the only guy who used Mouse2 as forward. I had a bunch of friends that introduced me to my first fps in Half-Life deathmatch many years ago and whenever I played, I just couldn't get a feel for a good config until one of my friends showed me his "home row" technique which is very similar to my setup. I do like it, but I've got 1 friend who thinks I'd benifit from standard controls. I've yet to try it, simply because I love this setup and it's hard to switch now that I've stuck with it for 3 years.

Atleast my logitech cordless optical has 1 button on the side and I couldn't bind it. But now that I formated my comp and didn't install the drivers it works fine. I'm pretty sure it has to do with the logitech windows xp drivers. Oh well now it works perfectly (well if I bind it)...
