Install, set your refresh rate for each resolution - fixed.
If all runs smooth by all means keep v-sync on, if your fps are dipping terribly, turn it off in your opengl properties.

You don't need to "crack" winXP, and windowsXP DOES allow for more then 60FPS in OpenGL....
First off, check if V-Sync (vertical Sync. It's a feature that times your monitor's refresh rate and your FPS into syncronisation (spelling?), so that the game runs with the outmost fluidity (it prevents artifacts from appearing)).
-Right Click on your desktop
-Click on "Properties"
-Go to the "Settings" tab
-Click on the "Advanced" button
-Now click on your videocards tab (ie GeForce4 ti 4200)
-Select "OpenGL settings"
-Where it says V-Sync, select "ALWAYS OFF"
Now, you will need to access your Natural Selection console by pressing the "tilde key" (the one under the "Esc" key and to the left of the "1" key) when your in a game, or by selecting the "Console" options in the main natural selection menu. Then you need to type:
"fps_max 99" (without the ")
and then you type:
"fps_modem 99" (without the ")
Now, to make sure the game is running at more then 60fps, type the following command in the console:
"cl_showfps 1" (without the ")
DO NOT USE CL_SHOWFPS 2 or CL_SHOWFPS 3 (they take up alot of ressources to draw the graphs and they WILL cut down on your FPS counts by a considerable amount). Once you have typed the "cl_showfps 1" command, get rid of your console by pressing the Tilde key. A small FPS indicator should be in the TOP LEFT corner of your screen (its only a 2 digit counter).
If the fps is still stuck at 59-60, then download newer drivers (go to the nvidia.com website and download the latest Detonator drivers, OR go to www.guru3d.com and download previous detonator drivers). You can also grab the "refresh rate fix" (NOT A HACK, a fix) from www.guru3d.com if I remember correctly.
Also, UPDATE YOUR WINDOWSXP!!! Most of the times, problems like this will be solved by downloading software packs and patches.
I have windowsXP and I NEVER had this problem, so it doesnt mean you need the "hack" right away.
Try the above things first.

I'm not sure which one it was because I tried all of them, but I went from an average of 15fps and less than 10 in a firefight to an average of 60-70fps.
However, when I get into fights taht last for over a couple minutes, I am playing in a game for an extended period of time or the comm is turret spamming and there are millions of them I might sink down to the 30's for a little while. Is there any way to stop this?
(I am running a 56K connection, could this be the problem?)
