What bugs me in this SS is that Doublejump is not in the game anymore with Build 250.
Please verify your game integrity. -
Uninstall NS2 and delete the NS2 folder in C:\Program Files (x86)\Steam\steamapps\common. Re Download and install the game.
This should get you playing again. -
Well with that kind of hardware you are bound to have fps problems.
Your Gfx card is low, and the CPU is not really better.
Before 249 you would have gotten max 7 fps or even less. -

Could be a problem with the servers performance.
Be sure to look at net_stats if there is less than 30 tickrate from the server -
You can't identify the faulty stick with the memory adress, you would have to test each on their own.

You can try it but you should check with memtest to be sure you've taken the right one.
If you got the right your problem should be gone -

run diskmgmt.msc and look for Harddisk4 the partition name should be there too

Try cleaning your Gfx card cooling system, if it's dirty I had those problems on a dying 6970 as well.

It will decrease the lifetime of the SSD, because an SSD has a set number of write operations in each sector.
After those were reached you can only read those sectors, so defragmenting is not good for SSD's. -
Harddisk4 should mean your 5th HD.
I have looked a bit and seen on some websites that the paging operations refer to caching problems. -
That is a defect sector scan if the data is corrupted by frequency issues it won't show up there because this test just looks for physically damaged sectors on your HD.

Afaik the SATA Bridge chip is connected to the PCI-X Hardware interface, so that if you increase the frequency of PCI-X the SATA Bridge chip passive with it.
Increasing BLCK increases PCI-X freq and so on.

Edit: Not …Well got the new card already and got to say it's much less noticable than before (AA enabled)
But still you can see it where the tiles or verts in the level design connect.
