Most of the problems you describe could be easily fixed if there was a reasonable moderator or admin present on the majority of the servers.

All I ever get are German servers filled with 15-17 mutes who seem to have invented (and perfected) TOTAL RADIO SILENCE.
Still crops up occasionally. Can't foretell if it decides to follow vsync settings or not.
My usual workaround is to switch to double buffered -> apply -> triple buffered -> apply.
