Depends on how you're creating the face. If you're using the rectangle tool but it's not showing up - @SamusDroid is correct and you need to hit Ctrl+F to flip the face…

I'm assuming you mean something more than the Weld Nearby Vertices (Ctrl+W) or Weld Selected Vertices (Ctrl+alt+W). Are you talking about welding models to geo or something crazy?

MAP IS UPDATED! File size is down to around 50MB - which compresses on Workshop to around 16MB it says. I haven't been able to test it yet on a server, to see if new players can join yet. My apologies - I left .psd files in that Rudy sent.

This is incorrect. Playing on one of your servers last night I was watching tick spike down below 10 at times, similar to how a sudden ping spike would feel. But it's not only your servers and the issue didn't exist in 248.
