It's all to do with how far away we are from the rest of the world. That's why it is so expensive. And it will always be that way. Too make it easy to understand, lets say they could invest in a cable that runs from Australia to overseas (a long freaking way) which services Aus's population of 22 million people. However another cable could be put through Japan that services 127 million of people and is only thousands of kms long. Shows you how much work has to be put into servicing such a small (relative) number of people. More of an analogy then how it exactly is, but helps you get the picture.
With that said it is ALOT better now compared to what it used to be.

Doesn't mean they will be willing to let go of their bandwidth caps. They already have everyone accepting silly caps as normal. All they need to do is more for a higher cap. I'm in America and the "High Speed Unlimited" line I have cuts my speed in half if I exceed a daily quota.

Quite possible. But that would mean, that your ISP is screwing you out of bandwidth, if you are not running IPTV 24/7. And even then my calculation stands non the less :P

You'd have to ask Necro, I'm including him as one of the possibles. He does love his anime and manga, it wouldn't surprise me if he gets 1TB a month.
Each patch contains more than just the code based fixes listed, models are updated, skins redone, new model assets with every patch, new sounds, additional animations.

the answer to your question is no, only games that are to be <b>published in australia</b> (read: retail) require review by the classification board, who would classify it from G, PG, M15+, MA15+, R18+. Unfortunately the classification standards for games and films are different - games currently lack an R18+ rating. Not relevant to NS2 however (not retail) as it is on the internet and it cannot be policed.
Also yes, we have quotas. Although Scorpy said that the average usage of the average Australian is 3gb/month, I don't think he was implying that that was his internet plan's download usage. Big companies like to rip people off by selling them 3gb quotas for $20/mo, that's all. I'm on an up-to-8mbit connection (moved house to a place without ADSL2 available), and I get 45gb/90gb on/off-peak, for $85/mo. Prices are pretty steep for me. Regardless, because I generally use a lot of bandwidth each month, I still control and ration my usage. 700mb is actually quite large for anyone with a download limit less than 50gb/mo.
Also, iiNet is ######' terrible. Wise up and get on Amnet (if you live in WA).
