Piercings use a tapered, bladed needle, as opposed to being a puncture like a normal injection or like a tattoo, it slices through the flesh.
I've not been inked yet, but i know a fair amount of people that have and also a few that do it, from what i've heard, with the exception of certain places where like those where the skin is tight over the bone, the spine, hip, etc, it's a relatively painless experience. But then again, tattoos take hours to do whereas a piercing is over in seconds.
