Does the game come with any "safe mode" or similar option? If not, is there any way to disable features such as sound or joysticks to determine what the problem isn't coming from?

Go to Call of Duty on the start->programs bar thing, make shortcuts for both, and then run it through there.

To get rid of the lag, I'd try running it a few more times, but don't skip through the intro. The 3rd and final intro movie is a huge one, so its not like its boring. If you make it all the way through so you can play, quit, and try again without waiting through intros.

To get through the intros(Q3 has a problem with the esc key which most games use to go through intro movies and stuff), just left click. Should skip 1 intro at a time.

Make sure your card is fully supported by the game. I glanced over the Readme files
for you but saw no mention of your card. (Have a look incase I missed something)
Are you using up-to-date nVIDIA drivers? Was the MX 420 released before or after
the 440? Because I used to use an MX 440 and it was pretty limited.
It struggled alot with Ghost Recon (15-20 fps even on the training course).
So try newer drivers, if not then try and get your
hands on a newer video card, even if its just for a few hours. At least that way you're
narrowing down on what could be causing the issues.

Remember that the engine this game uses is far from perfect.
I played it through with a RADEON 9000 and the newest drivers and
I still got weird vanishing water. Which made the level a complete nightmare to play.

Good luck with that as its a good game.

some radeon 9x00 users can't play the game _at all_ with the newest drivers, the game freezes after a few minutes of playing (or worse..)

Why can't all graphics cards live together in harmony and all WORK?
